The OMOP ETL Worksheet
A working checklist for taking a source database through to an OMOP CDM instance you can defend, with a place to record the decision behind every step.
Nothing you type leaves this page. Use Save file to keep your progress, and Load file to pick it back up on another day or another machine.
Project details
These entries head every file this worksheet writes, so the plan you hand to a collaborator says which site, which source, and which vocabulary release it describes.
Your plan so far

Where the steps come from
The stage order and task list follow the ETL workflow used by OHDSI network sites. The tools named in each stage are documented here:
- WhiteRabbit and Rabbit-In-a-Hat, for source database profiling and the mapping document
- OMOP Common Data Model specification, including the Imaging extension
- Achilles, Data Quality Dashboard, and Ares, for characterization and data quality reporting
- HADES and ATLAS with WebAPI, for the analytic layer
- Themis, for the CDM conventions your documentation is checked against
- ATHENA, for vocabulary downloads and release dates
- OHNLP, for pulling structured data out of clinical text
